Premiums, balance-billed charges, health care this plan doesn’t cover, expenses in excess of usual, customary and reasonable (UCR), penalties for failure to pre-authorize hospital admissions.

What services must be pre-authorized?
All in-patient hospital admissions (emergency admissions must be pre-authorized within 48 hours), acupuncture, home health care, hospice, organ transplants and skilled nursing facility admissions.
